Appointments
All patients require an appointment. Walk-ins are not encouraged.

There are 3 levels of appointment: 
     1. Emergent: An illness and/or injury that requires same day evaluation and  possible  Emergency Department treatment.
     2. Urgent: An illness which should be evaluated and treated within 24 hours.
     3. Routine: A physical or follow-up examination which can be scheduled within 30 days. 
  • Please arrive 20 minutes early for your appointment to fill out all necessary paper work.
  • You should also bring any medications that your child is currently taking
  • Make sure you have your current insurance card for the receptionist.We will scan your card, take a photo of your child and import any paperwork into the electronic medical record.
  • Established patients should present your current insurance card at check-in and inform us of any new phone number or address changes..
  • Our staff will verify coverage and deductible status. We do accept most commercial insurances. Every attempt will be made to verify your insurance benefits and eligibility prior to your appointment. In the event we are unable to do so, the guarantor will be responsible for payment at the time of service. You will be given paper work to submit to your insurance company for reimbursement.
  • All deductibles, co-payments, co-insurances and unpaid balances are due prior to seeing the doctor. Unpaid balances are subject to a $15.00 billing fee. We offer discounts to our patients that do not have insurance. Please contact the office manager to discuss the details. Patients receiving "private-pay" discounts cannot later submit bills for insurance payment (this considered insurance fraud). Installment payment arrangements can be made with the office manager.
  • Missed appointments without 24 hour notice will be subject to a $25.00 cancellation fee.  Fees must be paid prior to rescheduling appointment.
